INTEGRATED WORK-FOCUSED HEALTHCARE IN CARDIOVASCULAR DISEASE DEVELOPMENT OF A DIGITAL PLATFORM TO ENHANCE INFORMATION EXCHANGE AND COMMUNICATION

Introduction: Insufficient collaboration between work-focused and curative care sectors in the Netherlands poses challenges for addressing the aging workforce and increasing chronic diseases like cardiovascular disease (CVD). While regional networked care supported by digital platform solutions has been endorsed by governmental authorities under the recent Dutch integral healthcare agreement, there is still a lack of insight regarding design and implementation of such a platform across healthcare sectors. This study aims to develop a cross-sectoral digital platform that aligns with stakeholders' requirements and creates an integrated care network for work-focused healthcare in CVD.
Methods: Employing a mixed-method approach, this study incorporates qualitative observations, semi-structured interviews, and questionnaires. It is conducted in three stages: 1) iterative development monitoring with stakeholders engaged in the integral work-focused care trajectory, collaborating on platform creation; 2) exploring design opportunities and limitations for future digital support systems in cross-sectoral network care initiatives; and 3) evaluating adoption and usability of the digital platform for an integrated work-focused care network for people with CVD.
Results: Currently, N=19 stakeholders are included in the co-creation process for the iterative development of the digital platform. The preliminary version of the platform allows for information requests, viewing involved stakeholders, and requesting multidisciplinary consultations. The platform is expected to be completed by the time of the conference and will undergo a pilot phase in late 2023.
Discussion: Co-creation shows promise in meeting stakeholders' demands and ensuring sustainable implementation of work-focused healthcare solutions.
Conclusion: Regional work-focused care networks can enhance integrated work-focused healthcare, promoting sustainable employability and preventing long-term absenteeism.
