Description
The large increase in the number of older adults in the coming years requires a far-sighted approach to general practice care. The Amsterdam General Practitioners Alliance has the ambition to improve (integrated) care for vulnerable elderly people. To achieve this, the alliance is committed to 'Beter Oud in Amsterdam', with a step-by-step approach and deployment of the Practice Nurse for older Persons in almost all GP practices in Amsterdam by 2024.
