CT perfusion for acute ischemic stroke: Vendor-specific summary maps, motion correction and application of time invariant CTA

Abstract

Computed Tomography Perfusion (CTP) imaging, together with non contrast CT and CTA build a CT examination workflow for acute ischemic stroke patients. In CTP analysis, areas with brain perfusion defects can be detected immediately after the onset of clinical symptoms. In this thesis book, we introduce the basic principal of CT Perfusion and its role in diagnosis of acute ischemic stroke management, followed by discussion about the pitfalls that arise in the use of CTP related to two issues: lack of standardization and head movement during acquisition. Possible solutions offered in this thesis are described. We first qualitatively and quantitatively assessed the extent, frequency and severity of such motion during CTP acquisition in our patient database. It is followed by studying the effect of head movement during acquisition on the accuracy of CTP analysis by using digital head phantom data. Furthermore we propose an automated method for the selection of unsuitable CTP data with excessive movement and also a correction method for complete compensation of head movement during CTP acquisition by using 3D registration of the CTP dataset with non contrast CT data. In addition, the use of time invariant CTA (tiCTA) generated from CTP images is introduced. This study emphasizes the wide usability of CTP and tiCTA and generates options for better CT workflow for acute ischemic stroke patient.
