Abstract

OBJECTIVE: To compare community pharmacy medication records that are obtained by fax or downloaded through two different Ozis applications (electronic exchange of medication-related information through open source information systems using an Euroned link or a Microbais link). DESIGN AND METHODS: An observational study was conducted in January-February 2009. Patients who used at least one drug and from whom all three medication records (fax, Euroned and Microbais link) could be obtained were included. A fax of the last six months was requested from the community pharmacy upon hospital admission. A six months medication record was also printed through a web-based Ozis application (the Euroned link) or downloaded (the Microbais link) into the hospital's computerized physician order entry system. The information of all three medication records was combined and considered the gold standard. All medication records were compared with the gold standard regarding the presence of drugs, user information, allergies, contraindications and theoretical start and stop dates. RESULTS: In this study 149 patients were included. The highest number of drugs was presented by the Euroned medication records (99.0%), followed by the fax (98.4%) and the Microbais medication records (96.8%). The fax contained the most complete user information, allergies, contraindications and start and stop dates. Medication records obtained through Ozis applications could fail to provide user information (such as dosage) or provided incomplete information. The Microbais link could not provide information regarding allergies and contraindications. CONCLUSION: The three medication records provided inconsistent medication-related information. In general faxed medication records are the most complete.
