Better at home with General Practitioner and Specialist Elderly Care Medicine

It is necessary to improve the availability of expertise of the specialist in geriatric medicine (SO) to the general practitioner (practice) in order to provide good medical care to elderly people living at home with complex multi-problems. Research shows that deployment of the SO leads to better quality of life. It also saves costs: the elderly stay at home longer and use less hospital care. Research also shows large differences in working methods and that good, unambiguous funding is lacking nationwide. Therefore, there is a need for mutual exchange of experiences and knowledge, in order to achieve more unity in working methods and a clear, appropriate form of financing. By exchanging existing experiences and knowledge, supplemented where necessary with new knowledge to be gathered, this project will lead to a commonly shared working method with appropriate financing form. All relevant parties will be involved (patients, care providers, administrators, health insurers).
