Citizen-driven health care system performance: Studies on care experiences and engagement

Abstract

In Europe, many stakeholders in health care systems are motivated to strengthen inclusive governance mechanisms by adopting collaborative approaches to responding to citizens’ voice. This has led health care systems to start measuring outcomes and experiences of care from the perspective of health care service users, notably by using patient-reported experience measures (PREMs). This thesis addresses the problems of how knowledge about citizens’ care needs, preferences, and experiences is created and translated into actions towards strengthening the responsiveness and people-centeredness of health care systems.
This thesis consists of six studies zooming in on different European countries. In Part I, survey data are used to measure care experiences (Chapter 1) and preferences for attributes thereof (Chapter 2) among users of outpatient care in Hungary. Additionally, the relationship between digital health literacy and care experiences is explored in the study described in Chapter 3. In Part II, the study described in Chapter 4 focuses on how and for what purpose health care insurers in high-income economies use patient-reported data. After that, two citizen engagement applications are explored, notably the engagement of citizens in health care procurement decision-making by health care insurers in the Netherlands (Chapter 5) and how citizens were engaged in developing the first health system performance assessment (HSPA) framework for Ireland (Chapter 6). The findings of this thesis can support health care system actors across levels of the system to learn from the experiences of others and better coordinate efforts and resources in support of citizen-driven health care system performance.
