Abstract

The obstructive sleep apnea syndrome (OSAS) is a common problem in nowadays society and is characterized by symptoms, such as apnea's during sleep, snoring and hyper somnolence. Due to the cardiovascular risks, it is of major importance that patients with OSAS get treated. Worldwide CPAP is the standard treatment, but has a low long term compliance rate.1 Also surgery, such as uvulopala-topharyngoplasty (UPPP), is frequently performed but with less satisfying results. Therefore recently more attention has been drawn towards multilevel surgery in which the obstruction in the upper airway is treated at more levels at the same time.2 In this study 15 patients undergoing multilevel surgery consisting of UPPP, interstitial coagulation of the base of tongue and hyoidothyroidpexia, were prospectively evaluated, after failing CPAP therapy. Analysis of the data showed that both the rate of apnea's reduced with 65% and the subjective symptoms improved after surgery. Conclusively, this study shows that multilevel surgery in patients with OSAS has a high success rate, both subjectively and objectively. Therefore this surgical procedure seems a good alternative for patients who fail CPAP therapy.
