The following footnote has been added to Table 1: ‘‡Both patients had a high-grade neuroendocrine neoplasm with a Ki-index of >20% or >20 mitoses per 10 high power fields, which were thus classified as euroendocrine carcinoma (40).’ The footnote numbering in this table has also been adjusted. Reference 40 has been added. The word ‘predominantly’ has been added to the second sentence of the Discussion section.
