abstract = "A safety management system for patient safety was designed for use in the intensive care department at the Amsterdam Medical Centre. The management system is based upon design experience from the chemical industry. Since the rigid technical approach was difficult to implement in the hospital environment it had to be adapted to the local conditions. An analysis of the intensive care department and its staff provided insights in how to adapt the management system. The system that was developed has the characteristics of a safety management system as is common in the chemical industry but it makes use of risk instruments that are more familiar in hospitals. Since the system is its infancy it is much simpler than in the chemical industry. The medical and nursing staff are currently gaining experience and there are indications that the system operated effectively. Three bow-ties and two tripod-β analyses were performed. A rigorous evaluation is planned for the end of 2011.",
